Teleport wasn't even a spell in the early game.

When it was entered into the game it was considered very powerful, and therefore dangerous.

Having a secure place to teleport was far less likely in Greyhawk, since property wasn't by simple purchase like in a capitalist world, but depended on suffocating factors, like social status. You'll recall that your abode was achieved at a certain level, where, rules wise, people would be awed by you and follow you and the world would magically part for you and give you a propertied substance. That was written in almost that way, but what was really meant by Gary was that you couldn't really achieve this socially or in the power balance before then. Just keeping a room of your own in Greyhawk city, at a lower level, held similar problems. Even in an Inn, there are snoops and cleaning ladies. And to appear anywhere else, suddenly, perhaps harmed, perhaps with the glint of gold or bulging bags, was very dangerous.

Gary and Rob both took the familiarity chart much more seriously. You might THINK you're intimately familiar with a place and choose it as your destination, only to find yourself elsewhere and finally be driven to admit that the number of times you were there or how you became familiar with it was much different in your memory than it was in the judge's. And, Gary and Rob were judges because they could recall these things better. They were exemplars at war gaming even before role playing.

Teleporting only took one player out. The rest of your party might not adventure with you after that, so its use was very limited in a loyalty respect. Today, in 4E the spell will teleport out an entire party, no matter how far away from each other they are. Thus, in another ten years, answering this question will be even more difficult, since gamers will have been acculturated to a much more broad and giving spell. The old Greyhawk teleport may appear on the rules book page to be elementary, but it was far from it.

Accumulating newly introduced spells in the game wasn't exactly easy. You had to have enough intelligence, you had to find an instance of the spell, and then you had to decide it was of more utility than other spells to add to your books or to take on your adventure that day. So, it was very powerful, but was it worth taking.

There's also a problem with the very question. What do you mean by lost? Surely, there was a point when a greyhawk adventurer decided they were definitely lost, but that didn't really mean anything to them. What it meant was that they had no way out and were down to their last resources and needed out now. To even GET to that set of circumstances meant that you were alone, thus teleport had personal utility, and that you didn't need any of the other spells that teleport would have prevented you from taking. Sure, we make these decisions today, as well, so this isn't entirely unique as a point of fact, but consider this further:

Any adventurer into Greyhawk pretty much assumed that they were at a grave risk of getting lost. It was a large part of the risk. In fact, the dungeon was crazy in architecture, you had to preserve your mapping tools, and you had to take the time to map, which means a mere RUN of a couple feet could make you lost because you couldn't map it while constantly running. Plus, graphic skills were harder to find in a party back then. (If a TSR artist was in the party they were more likely to be the mapper) Today's adventurers are more grpahically inclined. Today's gamers use big grid maps and miniatures and the DMs draw the maps for them. And we're all most accustomed to the graphic interface of the computer, which alone increases our graphic mapping abilities. Wasn't that way back then! When adventuring in Greyhawk the pretense was that you probably were lost, perhaps even when you thought you knew where you were. After all, dim torchlight and a teleporter you ran through while being chased might not register. THere's a question as to how the magic of teleportation even works to obfuscate the differences between two points if it was put there as a trap by a crazed archmage. Today's gamer argues that the teleporters in the Maze of Zayene shouldn't be undetectable. How vain and stupid! We're a different set of gamers today, living with an entirely different set of assumptions.

So, if you did discover, inscribe, and one day memorize the teleport spell, and you were already separated from your party and had low resources and were under some threat, then you would have to have the time to focus on your target of teleportation and hope that you didn't get hit while doing so, and that you had enough time. You also had to be still for that time. Perhaps none of this in today's game is all that unlikely. However, in Greyhawk, everything was in a constant state of jam. You weren't adventuring in a light hearted world of freedom and capitalistic whim. Adventuring wasn't a matter of shopping for a little mystery and figuring it all out. It was a near death, bizarre immersion in the unknown and near impossible.

Another, most probably greater concern, was the idea of teleport being used to get back into an important place. But that was solved by the familiarity table. With today's game being what it is, a teleport exclusion would need to be written into Greyhawk dungeon, but easily done, considering the premise of the insane archmage. As I recall, TPing out of the original dungeon was only dampened or diverted in particular places and circumstances.
